Donation Form

Please make your donation of events, services, or new or gently used items (antiques welcome) before February 21 to be included in the catalog.

Be sure to include your phone number in case we have questions about your donation, and remember to hit the submit button at the bottom of the form when you are done.

You should see a confirmation screen summarizing your donation after you click the submit button. If you need to make corrections, just resubmit with a note in the description box or send an e-mail to webmaster@ouuc.org.
